19. Besonderheiten von Haptenen und Allergenen bei Spättypreaktionen

Zusammenfassung

Die allergische Kontaktdermatitis (AKD) ist eine T-Zell-vermittelte Hauterkrankung und betrifft 15–20 % der Bevölkerung. Der klinisch relevanten Auslösephase geht eine Sensibilisierungsphase voraus. Auslöser der AKD sind Kontaktallergene. Hierbei handelt es sich um organische Chemikalien oder Metallsalze, die nach Penetration der Haut direkt kovalent oder als Komplexe an Proteine binden. Grundlage für die Auslösung einer Immunantwort ist die starke Aktivierung des natürlichen Immunsystems durch Kontaktallergene. Dem entgegen wirken regulatorische Faktoren von T-Zellen. Diese T-Zell-vermittelte Immunantwort wird durch dendritische Zellen orchestriert. Arbeiten im murinen System legen nahe, dass Langerhans-Zellen eher regulatorische T-Zell-Antworten stimulieren und dermale dendritische Zellen für die Programmierung von proinflammatorischen, ekzemauslösenden T-Zellen verantwortlich sind. Aus den Ergebnissen dieser Arbeiten lassen sich nunmehr wertvolle therapeutische Ansätze ableiten.
