Bildgebende Verfahren bei Riesenzellarteriitis (M. Horton)

Zusammenfassung

In der vorliegenden Übersichtsarbeit werden die Vor- und Nachteile verschiedener bildgebender Verfahren bei Riesenzellarteriitis aufgezeigt.
Die Farbduplexsonographie ist kostengünstig und mit vergleichsweise wenig technischem Aufwand durchführbar. Sie ist jedoch von der Erfahrung des Ultraschalluntersuchers abhängig. Mitunter können entzündliche von artherosklerotischen Wandveränderungen nicht sicher differenziert werden.
Die Positronenemissionstomographie mit dem Glukoseanalogon 18F-fluoro-2-deoxy-D-glukose kann den extrakraniellen Befall der großen Arterien sehr sensitiv nachweisen. Dieses Verfahren kann jedoch keine Aussage über die oberflächlichen Temporalarterien liefern.
Mit der hochauflösenden, untersucherunabhängigen MRT können die oberflächlichen Kopfarterien artefaktfrei dargestellt und beurteilt werden. Gleichzeitig kann der extrakranielle Befall detailliert bewertet werden. Derzeit bestehen noch wenige monozentrische Erfahrungen mit diesem sehr viel versprechenden aber technisch aufwändigeren Verfahren. Momentan wird eine multizentrische Vergleichsstudie geplant.
