17. Biopsychosoziale Krankheitsmodelle

Zusammenfassung

Die Bedeutung psychologischer und sozialer Anteile an der Aufrechterhaltung von Rücken- und Nackenschmerzen wurde in den vergangenen 30 Jahren eindrucksvoll belegt. Dennoch werden selbst bei Patienten, bei denen psychosoziale Befunde festgestellt werden können, primär organbezogene Inhalte angesprochen. Der fehlende Transfer in die klinische Praxis gilt ebenso für die Aus- und Weiterbildung verschiedener Gesundheitsberufe, die an der breiten Versorgung beteiligt sind. Wichtig scheint uns hier zunächst auf der Sachebene, dass die Vielfalt möglicher psychosozialer Faktoren in plausible, leicht verständliche und damit gut vermittelbare Modellvorstellungen integriert wird und Wechselwirkungen mit biomedizinischen Befunden verdeutlicht werden. In diesem Beitrag schlagen wir ein Basismodell vor, das einheitlich von verschiedenen Gesundheitsberufen in kurzen Gesprächsabschnitten vermittelt und, bei Bedarf, durch spezifische Modelle ergänzt werden kann. Basierend auf einer zuverlässigen psychosozialen Diagnostik sollten, vor allem beim chronischen Schmerz, Anbieter eines multiprofessionellen Behandlungsverbunds möglichst identische Krankheitsmodelle verwenden, um sich gegenseitig sinnvoll ergänzen zu können…
