Zusammenfassung

Psychotherapie bei bipolaren Störungen war bis vor 20 Jahren ein Thema, das kaum Erwähnung fand. Dieses Kapitel versucht einleitend durch eine genaue Darstellung der Symptomatik und Diagnostik deutlich zu machen, wie bereits die Diagnosestellung in vielen Fällen durch falsche Annahmen behindert wird. Oft stehen Depressionen im Vordergrund und hypomane und manische Episoden werden übersehen. Das vorgestellte integrative ätiologische Modell sieht in einer Störung der Verhaltensaktivierung das Grundproblem bipolarer Störungen und verdeutlicht, welche Rolle persönliche Defizite und Ressourcen sowie Stress spielen. Störungsspezifische diagnostische Instrumente werden insbesondere im Hinblick auf die Therapie vorgestellt. Das konkrete Vorgehen bei einer Rezidivprophylaxe wird skizziert und auf typische Probleme eingegangen. Der Fokus dieses Kapitels liegt dabei auf manischen Zuständen. Die empirische Evidenz für die Wirksamkeit psychotherapeutischer Intervention wird vorgestellt und abschließend auch erwähnt, wo offene Fragen sind und was zu tun ist, um die Situation für die Betroffenen zu verbessern.
