Calciphylaxie

Zusammenfassung

Hintergrund

Die Calciphylaxie ist eine seltene, jedoch mit hoher Morbidität und Mortalität assoziierte Erkrankung, die hauptsächlich bei Dialysepatienten und Patienten mit fortgeschrittener Nierenerkrankung auftritt. Sie ist gekennzeichnet durch äußerst schmerzhafte, ischämisch bedingte und mitunter nekrotisierende Ulzerationen der Haut, histomorphologisch findet man ausgeprägte Kalzifikationen der Media kutaner Arteriolen, von Nervenscheiden und im umgebenden Fettgewebe. Eine differenzialdiagnostische Abgrenzung zu vaskulitisch bedingten Hautmanifestationen, zur Pyoderma gangraenosum, zu diabetischen Ulzerationen oder Cholesterinembolien kann in Einzelfällen schwierig sein. Bis vor wenigen Jahren bestand die Auffassung, dass die Calciphylaxie strikt an das Vorhandensein eines schweren Hyperparathyreoidismus gekoppelt sein muss. Neue Registerdaten stellen diese Wahrnehmung jedoch deutlich in Frage. Vielmehr scheinen Faktoren wie eine positive Kalzium- und Phosphatbilanz, ein adynamer Knochen, Entzündungsprozesse, Verkalkungsinhibitormangel und Vitamin-K-Antagonismus als Risikofaktoren eine Rolle zu spielen.

Schlussfolgerung

Neue therapeutische Ansatzpunkte (z. B. Cinacalcet, Bisphosphonate, Natrium-Thiosulfat, Vitamin K2) sind in Erprobung. Aufgrund der Seltenheit der Erkrankung existieren derzeit aber keine randomisiert-kontrollierten Studien hinsichtlich dieser Behandlungsansätze.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Prof. Dr. med. Gregor Antoniadis Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
