In this issue of Hernia, Aufenacker et al. describe technical changes in the operative therapy of inguinal hernia in the Amsterdam region 1994–2001. The authors conclude a decreased rate of operation for recurrence due to both the rising use of mesh prostheses and an improved supervision of surgical residents. Though excellent results repeatedly have been published in personal series for various techniques, the rate of re-interventions in the routine surgery of a defined region whole supply area should be the most decisive point to select best standard techniques in the management of inguinal hernia.
