Chemotherapeutische Konzepte bei Knochensarkomen

Zusammenfassung

Hintergrund

Ewing- und Osteosarkome sind die häufigsten auf Chemotherapie sensitiven Knochensarkome.

Ziel

Es werden die Entwicklung, wichtige aktuelle Chemotherapiekonzepte und künftige Behandlungsstrategien bei Ewing- und Osteosarkomen dargestellt.

Methoden

Es erfolgte eine Suche von Artikeln („Osteosarcoma“ und „Ewing Sarcoma“) via PubMed und anfolgend eine synoptische Darstellung wichtiger Ergebnisse.

Ergebnisse

Beim Osteosarkom wie beim Ewing-Sarkom sind mit konventionellen multimodalen Therapiekonzepten aus Chemotherapie und Lokaltherapie Plateaus im Behandlungserfolg, mit 5-Jahres-Überlebensraten von etwa 70 %, erreicht worden.

Diskussion

Um neue therapeutische Zielstrukturen zu identifizieren, sind bessere Einblicke in die molekulare Biologie und Genetik sowie in die Interaktion der Sarkomzellen mit dem Immunsystem und der Tumormikroumgebung notwendig. Bei der Durchführung prospektiver, randomisierter Studien zur Evaluierung der Wirksamkeit neuer Therapien sind wegen der Seltenheit der Erkrankungen multinationale Kooperationen anzustreben.
