Die Laryngektomie – noch zeitgemäß?

Zusammenfassung

Die Kehlkopfentfernung (Laryngektomie) bietet für Patienten mit fortgeschrittenen Larynx- und Hypopharynxkarzinomen ohne Fernmetastasen einen kurativen Ansatz. Insbesondere bei Vorliegen eines cT4a-Karzinoms ist die Laryngektomie der primären Radio(chemo)therapie prognostisch (retrospektiv) überlegen. Weitere wichtige Indikationen für eine Laryngektomie stellen eine tumorbedingte Larynxdysfunktion wie Schluckunfähigkeit bzw. Aspirationsneigung und ein Rezidiv nach vorausgegangener Radio(chemo)therapie dar. Das operative Verfahren ist hoch standardisiert und ausreichend sicher. Der mit einer Laryngektomie einhergehende Stimmverlust ist durch eine funktionelle Rehabilitation (Ruktus, Stimmprothese) zu kompensieren. Die vorliegende Arbeit verschafft einen Überblick hinsichtlich Indikation, präoperativer Diagnostik, operativem Verfahren, möglichen Komplikationen, alternativer Behandlungsansätze sowie Rehabilitation und Prognose. Die Laryngektomie gehört auch weiterhin zum bewährten Standardrepertoire der modernen Kopf-Hals-Onkologie.
