Zusammenfassung

Mit ca. 200.000 Neuerkrankungen in Deutschland pro Jahr hat die Versorgung von Leistenhernien eine hohe sozioökonomische Bedeutung. Für Klassifikation und Behandlung der Leistenhernien wurden 2009 von der European Hernia Society Empfehlungen herausgegeben. Primäres Ziel der operativen Versorgung ist die Rekonstruktion der Leistenkanalhinterwand. Hier konkurrieren anteriore und posteriore Operationsverfahren, welche in Naht- oder Netztechnik ausgeführt werden können. Aufgrund niedriger Rezidiv- und Komplikationsraten werden bereits bei primären Hernien meist leichtgewichtige, grobmaschige Kunststoffnetze implantiert. Laparoskopische Verfahren werden aufgrund niedriger Schmerzen und Arbeitsunfähigkeitsdauer zunehmend angewandt, sind jedoch mit einem spezifischen Eingriffsrisiko verbunden.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Prof. Dr. med. Gregor Antoniadis Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
