Chirurgische Strategien beim Zufallsbefund eines Appendixkarzinoids

Zusammenfassung

Appendixkarzinoide (neuroendokrine Tumoren [NET] der Appendix) sind seltene, auch heute noch fast ausnahmslos postoperativ beobachtete histologische Zufallsbefunde nach Appendektomie bei akuter Appendizitis oder im Rahmen anderer Baucheingriffe. Die meisten Appendixkarzinoide liegen in der Appendixspitze, sind unter 2 cm groß und metastasieren nicht, sodass erweiterte Nachoperationen selten erforderlich sind. Nachoperationen (Hemikolektomie rechts) sind mit einem ungleich höheren Risiko verbunden als die alleinige Appendektomie. Die Entscheidung über eine Nachoperation ist daher an histopathologische Zusatzinformationen gebunden, die über die alleinige Angabe von Lokalisation und Tumorgröße hinausgehen (Grading, Mesoappendixinfiltration, histologischer Typ). Da Appendixkarzinoide in bis zu 20% mir anderen Neoplasien assoziiert sind, kommt der Nachsorge nicht nur bezüglich des Primärtumors, sondern auch möglicher Zweittumoren besondere Bedeutung zu.

Prof. Dr. med. Gregor Antoniadis Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
