19. Chirurgische Therapieoptionen bei peritonealer Metastasierung

Zusammenfassung

Mit annähernd 1 Mio. Neuerkrankungen pro Jahr ist das Magenkarzinom weltweit die 5-häufigste Krebserkrankung. Von erheblicher Bedeutung ist dabei die geografische Region: Mehr als zwei Drittel der Patienten stammen aus sog. Entwicklungs- und Schwellenländern. Korea, Japan und die Mongolei verzeichnen die höchsten Inzidenzraten (Stewart u. Wild 2014). In Europa lässt sich ein Ost-West-Gefälle der Häufigkeit feststellen, das sich auch innerhalb Deutschlands bemerkbar macht. Seit mehreren Jahrzehnten wird hierzulande – genauso in anderen westlichen Industriestaaten – ein ständiger Rückgang von Inzidenz und Mortalität beobachtet: Das Robert-Koch-Institut registriert zwischen 1990 und 2004 einen Rückgang der altersstandardisierten Inzidenz bei Frauen um 38 % und bei Männern um 30 % (RKI 2016).

Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
