Chronisch-entzündliche Darmerkrankungen in der Schwangerschaft

Zusammenfassung

Morbus Crohn und Colitis ulcerosa sind die häufigsten Formen chronisch-entzündlicher Darmerkrankungen. Der Häufigkeitsgipfel dieser Erkrankung fällt mit dem Zeitraum der Fertilität zusammen. Demnach besitzen die präkonzeptionelle Beratung und medikamentöse Einstellung zur Minderung der Aktivitätsindices perikonzeptionell einen hohen Stellenwert. Die medikamentöse Therapie ist grundsätzlich der Krankheitsaktivität anzupassen, wobei eine First-line- und eine Second-line-Therapie zu unterscheiden sind.

Für die Prognose der Erkrankungen in der Gravidität sind weiterhin die intensive Schwangerenberatung und Ernährungskonzeption von Bedeutung. Die Inzidenz von Komplikationen—Spontanaborte, Frühgeburten, fetale Wachstumsretardierung und Fehlentwicklungen—ist erhöht. Die chirurgische Intervention in der Schwangerschaft sollte Ausnahmen vorbehalten werden.
