Die Therapie der chronischen Niereninsuffizienz ist eine Herausforderung. Wir haben uns in der klinischen Praxis daran gewöhnt, dass ab einer bestimmten chronischen Schädigung der Niere Mechanismen einsetzen, welche zu fortschreitendem Verlust der Nierenfunktion führen. Bislang werden diese Mechanismen v. a. durch die Behandlung weiterer schädigender Faktoren wie Bluthochdruck sowie metabolische Störungen wie Diabetes und/oder Hypercholesterinämie und Hyperurikämie behandelt. Es liegen zahlreiche Hypothesen zur Progredienz der chronischen Schädigung bei Niereninsuffizienz vor. Allen gemeinsam ist, dass über die initiale Schädigung und weitere Beanspruchung des Nierengewebes eine chronische Inflammation, möglicherweise schubartig verlaufend, mit Aktivierung interstitieller Fibroplasten und zunehmender Fibrosierung der Niere, eine entscheidende Komponente darstellt. Diese zellulären Mechanismen der Schädigung mit nachfolgender Inflammation und Aktivierung der Fibrose sind die Grundlage zahlreicher Bemühungen, neue Medikamente für die Behandlung der chronischen Niereninsuffizienz zu finden. …
