Chronobiologie der idiopathischen Hypersomnie

Zusammenfassung

Mit einer geschätzten Prävalenz von 0,005 % ist die idiopathische Hypersomnie eine extrem seltene Erkrankung. Sie gehört zu der Gruppe der Hypersomnolenzerkrankungen zentralen Ursprungs mit dem krankheitsdefinierenden Symptom der exzessiven Tagesschläfrigkeit. Trotz verschiedener neuroendokriner, immunologischer und genetischer Erklärungsansätze, die auch Untersuchungen der Schlafhomöostase und der zirkadianen Regulation des Schlafes einschlossen, konnten in den letzten Jahren zwar verschiedene neue pathophysiologische Aspekte der idiopathischen Hypersomnie aufgeklärt werden, diese erklären das Krankheitsbild jedoch weiterhin nicht hinreichend. Dieser Übersichtartikel gibt einen kurzen Überblick über die klinischen und epidemiologischen Daten sowie verschiedene pathophysiologische Konzepte. Hierbei wird insbesondere die Rolle der zirkadianen Rhythmusstörung als möglicher pathogenetischer Mechanismus der idiopathischen Hypersomnie herausgearbeitet.
