A 64-year-old male patient with impaired wound healing and sternal dehiscence due to mediastinitis after a coronary bypass surgery developed atrial fibrillation with symptomatic pauses up to 15 s. Due to the elevated risk of infection after implantation of a conventional pacemaker, we decided to use a leadless cardiac pacemaker (Micra™, Medtronic Inc., Minneapolis, MN, USA) instead. The implantation procedure went through without any complications, and the device could be successfully placed at the apicoseptal region of the right ventricle at first attempt.
