Comparison of a novel endoscopic suturing device to conventional endoscopic suturing device in a validated simulator: a randomized, crossover study (with video)

Abstract
Background and aims
Endoscopic suturing using a conventional suturing device has a steep learning curve. This study assesses the ease of use of a novel endoscopic suturing system compared to the conventional device using a validated simulator.
Methods
This randomized, crossover study included suturing experts and novices performing pursestring suturing on a transoral outlet reduction simulator. Subjects were randomized to use the novel or conventional device first. Simulator scores and workload using National Aeronautical and Space Administration Task Load Index were recorded.
Results
Ten subjects participated (4 experts, 6 novices). The novel device achieved higher simulator scores (179.3 ± 73.7 vs. 99.8 ± 94.1; P = 0.049) and lower workload (39.2 ± 15.9 vs. 70.5 ± 22.1; P = 0.002) than the conventional device. Novices demonstrated the greatest benefit.
Conclusion
This novel endoscopic suturing system improves ease of use and reduces workload, especially in endoscopists without prior suturing experience.
