Zusammenfassung

Klinisches/methodisches Problem

Das Prostatakarzinom ist die häufigste maligne Erkrankung und die zweithäufigste karzinombedingte Todesursache bei Männern in Deutschland. Eine sichere Diagnose und ein Staging mit präziser Bildgebung sind entscheidend, um den Patienten eine zielgenaue Behandlung zu bieten. Die Rolle der Nuklearmedizin bei der Diagnose des Prostatakarzinoms hat sich in den letzten Jahren durch die Verfügbarkeit der Hybridbildgebung mit prostataspezifischen Radiopharmaka rasant weiterentwickelt.

Radiologische Standardverfahren

Die Hybridbildgebung bietet eine höhere diagnostische Genauigkeit im Vergleich zur konventionellen Bildgebung und hat einen erheblichen Einfluss auf die klinische Behandlung. In der klinischen Anwendung wurden zahlreiche Radiotracer eingesetzt, wobei PSMA-Liganden am häufigsten verwendet werden.

Methodische Innovationen

Die Positronen-Emissions-Tomographie/Computertomographie mit dem prostataspezifischen Membranantigen (PSMA-PET/CT) und die PET/Magnetresonanztomographie (MRT) bieten im Vergleich zur konventionellen Bildgebung sowie zu anderen Radiotracern eine höhere Empfindlichkeit für den Nachweis von Lymphknoten- bzw. Fernmetastasen.

Leistungsfähigkeit

Aufgrund der hohen Genauigkeit für das primäre Staging beim Hochrisikoprostatakarzinom hat die PSMA-PET/CT Einzug in die neue S3-Leitlinie des Prostatakarzinoms gefunden.

Bewertung

Ziel dieses Artikels ist es, einen Überblick über den Einsatz der PET-Bildgebung in der Primärdiagnose des Prostatakarzinoms zu geben, die am häufigsten verwendeten Radiotracer vorzustellen und die Ergebnisse aktueller Studien zu diskutieren.

Bei Immuntherapien das erhöhte Thromboserisiko beachten

Unter modernen Systemtherapien versechsfacht sich das VTE-Risiko. Warum diese Daten relevant für die Behandlung krebsassoziierter Thrombosen sind, erläutert Prof. F. Langer im Interview. So kann es durch Immuntherapien zu inflammatorischen Syndromen z.B. im GI-Trakt kommen. Nebenwirkungen wie Durchfall oder Mukositis haben dann Einfluss auf die Wirksamkeit oraler Antikoagulantien. Aber auch in punkto Blutungsrisiko ist Vorsicht geboten. Wann hier bevorzugt NMH eingesetzt werden sollten, erläutert Prof. Langer im Interview.

CAT-Management ist ganz einfach – oder doch nicht?

Krebsassoziierte venöse Thromboembolien (CAT) haben in den vergangenen Jahren stetig zugenommen. Was hat der Anstieg mit modernen Antitumortherapien zu tun?
