Zusammenfassung

Die Ernährung ist der wichtigste Regulator des intestinalen Mikrobioms und seiner Diversität. Alle Makronährstoffe (Kohlenhydrate, Fette, Proteine) werden, soweit sie nicht im Dünndarm vollständig resorbiert werden, von den Darmbakterien, die vorwiegend im Dickdarm lokalisiert sind, metabolisiert. Diese bakterielle Metabolisierung im Darm modifiziert die Zusammensetzung und die Funktion des Mikrobioms mit der Zielsetzung, die Energie- und Substratgewinnung zu optimieren. Dabei sind kurzfristige, mittelfristige und langfristige Veränderungen des Mikrobioms durch diätetische Faktoren und Essgewohnheiten zu beobachten, denen unterschiedliche Mechanismen wie Substratlimitation und Epigenetik zugrunde liegen. Die Optimierung der Verdauung und Energiegewinnung ist bei drohender Mangelernährung von Vorteil, kann aber bei Nahrungsüberfluss nachteilig werden. So erklärt sich, warum das Darmmikrobiom in der Pathogenese von metabolischen Erkrankungen wie Adipositas, Fettlebererkrankung und Diabetes eine zentrale Rolle spielt.
