Das Deutsche Akanthamöbenkeratitis-Register

Erste Ergebnisse einer multizentrischen Erhebung

Zusammenfassung

Hintergrund und Ziele

Im September 2011 gründete die Sektion Kornea der Deutschen Ophthalmologischen Gesellschaft (DOG) das erste Akanthamöbenkeratitis-Register Deutschlands. Die Daten dieser multizentrischen Erhebung werden an der Universitätsaugenklinik Homburg/Saar gesammelt und ausgewertet. Unser Ziel ist es, einen Zwischenbericht zu präsentieren.

Patienten und Methoden

Es wurden die Patientendaten der letzten 10 Jahre mit einer gesicherten Akanthamöbenkeratitis gesichtet. Insgesamt wurden bislang 172 Patienten in unser Akanthamöbenregister aufgenommen, von denen derzeit 121 Patientendatensätze (60,2 % weiblich, Durchschnittsalter 41,3 Jahre) ausgewertet werden konnten. Es wurden folgende Daten erhoben: Datum des Symptombeginns, Datum und Methode der Diagnosestellung, initiale Diagnose, anamnestische Daten, Befunde bei Aufnahme und Follow-up, konservative und chirurgische Therapie. Einschlusskriterium für die Aufnahme in das Akanthamöbenkeratitis-Register ist die gesicherte Diagnose einer Akanthamöbenkeratitis mit mindestens einem der unten aufgeführten akzeptierten Verfahren.

Ergebnisse

Die Akanthamöbenkeratitis konnte in 55,3 % in der Histologie, in 25,6 % der Fälle mittels Polymerasekettenreaktion (PCR), in 20,4 % mit dem Konfokalmikroskop und in 15,5 % mittels In-vitro-Kultivierung nachgewiesen werden. Klinische Symptome oder Befunde bei Akanthamöbenkeratitis waren: Schmerzen 67,0 %, Ringinfiltrate in 53,4 %, pseudodendritiforme Epitheliopathie in 11,7 % sowie Keratoneuritis in 5,8 %. Bei 47,6 % der Fälle war die initiale Fehldiagnose Herpes-simplex-Virus-Keratitis, gefolgt von bakterieller Keratitis in 25,2 % und mykotischer Keratitis in 3,9 %. Bei 23,2 % der Augen wurde eine Akanthamöbenkeratitis initial korrekt diagnostiziert. Die Zeit vom Symptombeginn zur Diagnosestellung betrug 2,8 ± 4,0 (0 bis 23) Monate. Eine Dreifachtherapie mit Brolene, Lavasept und antibiotischen Augentropfen (wenigstens 5-mal/Tag) erfolgte bei 54,5 % (66) der Patienten. In 40,4 % der Fälle wurde eine perforierende Keratoplastik durchgeführt, davon in 18 Fällen in Kombination mit Kryotherapie der Kornea. Der Transplantatdurchmesser lag bei 7,9 ± 1,1 (3,5–11,0) mm. Der Endvisus (Snellen-Visus in 5 m) war in den beiden Gruppen mit (5/40 ± 5/25) und ohne (5/32 ± 5/25) Keratoplastik vergleichbar.

Schlussfolgerung

Bei der Akanthamöbenkeratitis handelt es sich um eine seltene und oft spät diagnostizierte Erkrankung. Zwei Drittel der Fälle wurden primär fehldiagnostiziert. Die Früherkennung der typischen Symptome ist für die Prognose ausschlaggebend. Alle Augenkliniken in Deutschland sind aufgefordert, retrospektiv und prospektiv möglichst alle gesicherten Fälle einer Akanthamöbenkeratitis dem Akanthamöbenkeratitis-Register zu melden (berthold.seitz@uks.eu), um adäquate und standardisierte Diagnose und Therapieschemata für dieses potenziell zur Enukleation führende Krankheitsbild zu entwickeln.
