Das Fachgebiet Anästhesie abseits der westlichen Medizin

Unter besonderer Berücksichtigung eigener Erfahrungen in der Demokratischen Republik Kongo und der Mongolei

Zusammenfassung

Während die Anästhesie in den letzten Jahrezehnten in den westlichen Nationen zu einem großen Fachgebiet herangewachsen ist, beschränkt sie sich in den Entwicklungs- und Schwellenländern zumeist noch auf ihr Kerngebiet, die perioperative Betreuung chirurgischer Patienten. Schlechte sozioökonomische Strukturen und eine mangelnde Finanzierung der Gesundheitssysteme bedingen einerseits eine hohe Krankheitsbelastung der Bevölkerung, andererseits einen hohen Anteil an Eigenfinanzierung der Gesundheitskosten durch die Patienten sowie inadäquate personelle, infrastrukturelle und materielle Ausstattung der meisten Krankenanstalten. Im Bereich der Anästhesie stellen unzureichende Ausbildungsstandards, der weit verbreitete Mangel an Sauerstoff, medizinischen Gassystemen und lokal einsetzbarer Medizintechnik wichtige limitierende Faktoren dar. Studien, die die Situation der Anästhesie in den Entwicklungs- und Schwellenländern näher evaluieren, werden dringend benötigt, um auf nationaler/internationaler sowie auf institutioneller und privater Ebene sinnvolle Hilfe zur Entwicklung der Anästhesie in den armen Ländern leisten zu können.
