In 1972, Abraham Vater described ampulla of Vater for the first time [
1]. While tumours arising from the ampulla are known as ampullary tumours, those arising within 2 cm of the ampulla of Vater are known as periampullary tumours. This definition also forms the basis of 8th edition of AJCC. Although morphologically periampullary tumours have been classified into pancreaticobiliary and intestinal type, histologically it shows a wider spectrum of tumours ranging from the most common adenocarcinomas to the rarest type like adenosquamous variety [
2]. Adenosquamous periampullary carcinoma has been described as a rare entity and so far 19 cases have been reported in English literature. The exact incidence is unknown and there is no consensus as how they should be managed and followed up. There is a great deal of variation in the criteria used for their diagnosis, and their histogenesis is still speculative. A diagnosis of adenosquamous carcinoma would not matter much to a surgeon as the surgical treatment remains pancreaticoduodenectomy, but it would be certainly pertinent to an oncologist or members of a multidisciplinary board, who are faced with the difficult choice of choosing between observation or adjuvant treatment in absence of guidelines. …
