Adipositastherapie – legale und illegale Arzneimittel und die Zukunft

Zusammenfassung

Adipositas und ihre Begleit- und Folgeerkrankungen stellen ein weltweit wachsendes Gesundheitsproblem dar. In Deutschland sind mindestens 15 Mio. Menschen von dieser Erkrankung betroffen. Die bisherigen Therapiekonzepte zur Lebensstiländerung zeigen bei den meisten Betroffenen nur begrenzte Wirksamkeit, was Ausmaß und Nachhaltigkeit der Gewichtsabnahme betrifft. Die wirksameren chirurgischen Interventionen müssen dagegen Menschen mit extremer Adipositas (Body-Mass-Index ≥40 kg/m2) vorbehalten bleiben. Daher wird seit Langem nach pharmakologischen Optionen für eine Unterstützung der Lebensstilmaßnahmen gesucht. Derzeit stehen mit Orlistat, Liraglutid und Amfepramon nur wenige Substanzen mit einer Reihe von Einschränkungen zur Verfügung. Neue Entwicklungen geben aber berechtigten Anlass zur Hoffnung, dass es in den kommenden Jahren zu einem Paradigmenwechsel in der Pharmakotherapie der Adipositas kommen könnte. Für viele alternative „Schlankheitsmittel“, insbesondere in Form von Nahrungsergänzungsmitteln, fehlt wissenschaftliche Evidenz bezüglich der Wirksamkeit bzw. sind die Effekte ohne klinische Relevanz.
