Akute Pankreatitis durch einen in das Duodenum prolabierten malignen Magenpolypen

Zusammenfassung

Polypen des Magens sind mit einer Inzidenz von 1,7–3,9% im Gegensatz zu Polypen des Kolons selten. Im Regelfall bleiben sie asymptomatisch und werden im Rahmen einer Ösophagogastroduodensokopie als Zufallsbefund entdeckt. In der Weltliteratur sind bisher lediglich drei Fälle einer durch einen in das Duodenum prolabierten Magenpolypen verursachten Pankreatitis beschrieben. Sofern eine endoskopische Abtragung nicht möglich ist, müssen nicht hyperplastische Polypen chirurgisch entfernt werden. Bei Nachweis eines Karzinoms in einem Polypen ist lediglich im Stadium des Frühkarzinoms vom Mukosatyp eine limitierte Operation vertretbar, ansonsten ist die Gastrektomie und radikale D2-Lymphadenektomie erforderlich.

Karpaltunnelsyndrom CME-Kurs

Prof. Dr. med. Gregor Antoniadis Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
