Zusammenfassung

Die Tako-Tsubo-Kardiomyopathie wurde erstmals vor 30 Jahren beschrieben. Das Krankheitsbild selbst ist also so lange bekannt, wie das Brugada-Syndrom. Seit 10 Jahren werden Registerdaten erhoben und publiziert. Die pathophysiologischen Mechanismen sind nicht geklärt, aber Studien der letzten Jahre haben Mechanismen beschrieben, die wahrscheinlich zum Tako-Tsubo-Syndrom (TTS) führen bzw. beitragen. Studien der letzten Jahre konnten zeigen, dass die Prognose schlechter sein kann als initial angenommen. Die akute und subakute Mortalität ist mit derjenigen des Myokardinfarktes vergleichbar. Ein emotionaler Trigger ohne weitere Risikofaktoren ist mit einer günstigen Prognose verbunden. Verschiedene Faktoren wie reduzierte linksventrikuläre Ejektionsfraktion (LV-EF), verzögerte LV-EF-Erholung, Linksschenkelblock, rechtsventrikuläre Beteiligung, höheres Alter, Tumorerkrankungen sind mit einer schlechten Prognose assoziiert. Eine spezifische Therapie ist nicht bekannt. Es gelten die gleichen Therapieprinzipien wie bei akutem Koronarsyndrom und akuter Herzinsuffizienz.
