22. Alloplastische Netze und ihre operative Entfernung bei Komplikationen

Zusammenfassung

Alloplastische Materialien werden zunehmend seit der Mitte der 1990er-Jahre zur Verstärkung von eigenem Gewebe bei Inkontinenz- und Deszensusoperationen verwendet, jedoch wurden mögliche zusätzliche Komplikationen wie Implantationstrauma, Fremdkörperreaktion, Infektion und Stabilitätsverlust wenig beachtet. Neben einer ausführlichen Beratung und Aufklärung über Komplikationen wird ein spezielles operatives Training im Umgang mit Fremdmaterialien, inklusive Risikomanagement empfohlen. Auch die Beachtung von Materialeigenschaften ist von immenser Bedeutung.
Im Fall von Komplikationen ist nach Klassifikation derselben und sorgfältiger Planung eine operative Entfernung des Materials notwendig. Einfach und vorteilhaft zeigten sich die transurethrale und vaginale Netzexzision. Dabei ist besonders auf eine gewebeschonende Präparation und engmaschige Nachsorge zu achten. Revisionsoperationen gehören in die Hände von Experten und müssen prospektiv in Studien und Registern dokumentiert werden.
