Zusammenfassung

Der Altershypogonadismus (Late-onset-Hypogonadismus) wird charakterisiert durch fortgeschrittenes Alter, niedrige Androgen-Serumspiegel (<8–12 nmol/l totales Testosteron) in Verbindung mit verschiedenen sexuellen und anderen körperlichen Beschwerden, kognitiven Einschränkungen und verringerter Lebensqualität. Die klinische Symptomatik ist jedoch nicht ausreichend deutlich definiert, die Symptome lassen sich am besten mithilfe verschiedener Fragebögen bewerten. Hinzu kommen diagnostisch eine Hormondiagnostik sowie die Suche nach Komorbiditäten. Sexuelle Störungen lassen sich per Schwellkörperinjektionstherapie, Phosphodiesterase-(PDE-)5-Inhibitoren oder auch einer penilen Prothese behandeln. Einer oft assoziierten Osteoporose sollte durch ausreichend Sport und Vitamin D/Kalzium begegnet werden. Bei manifestem Hypogonadismus mit klinischer Symptomatik ist eine Testosteronsubstitution indiziert; Kontraindikationen sind Prostatakarzinom, Mammakarzinom und eine unbehandelte Schlafapnoe. Zur Verfügung stehen orale, i.m. oder s.c. zu injizierende, transdermale (Gel, Pflaster) und bukkal zu applizierende Präparate.
