Die neue S1-Leitlinie „Geburtshilfliche Analgesie und Anästhesie“ – Vorstellung und Kommentar

Zusammenfassung

In den 682 deutschen Krankenhäusern mit geburtshilflichen Abteilungen (IGES G‑BA 2018) wurden 2019 778.100 Kinder geboren, die Sectio-Rate betrug 29,1 % [1]. Die geburtshilfliche Analgesie und Anästhesie ist obligater Bestandteil in der anästhesiologischen Facharzt-Weiterbildung und eine der anästhesiologischen Kernkompetenzen. Die 2., überarbeitete Empfehlung der Deutschen Gesellschaft für Anästhesiologie und Intensivmedizin (DGAI) und des Berufsverbandes Deutscher Anästhesisten (BDA) in Zusammenarbeit mit der Deutschen Gesellschaft für Gynäkologie und Geburtshilfe (DGGG) zur Durchführung von Analgesie und Anästhesieverfahren in der Geburtshilfe [2] war mittlerweile 11 Jahre alt – es bestanden der Wunsch und die Notwendigkeit, diese Empfehlungen für deutsche Anästhesisten zu aktualisieren. 18 Experten der geburtshilflichen Anästhesie aus Deutschland und der Schweiz waren in Abstimmung mit der DGGG an der Erstellung der S1-Leitlinie „Geburtshilfliche Analgesie und Anästhesie“ beteiligt [3], die sich der Aufarbeitung spezieller geburtshilflich-anästhesiologischer Fragestellungen widmeten. Die seit März 2020 publizierte S1-Leitlinie erfolgte in neuem Stil und entspricht erstmals den strukturierten Anforderungen der AWMF. Des Weiteren wurden, in Anlehnung an die „practice guidelines“ der amerikanischen Fachgesellschaften, erstmals die wissenschaftliche Evidenz und Expertenempfehlung mit einem Meinungsumfrageergebnis unter geburtshilflich interessierten und tätigen Anästhesisten kombiniert.
Neue Aspekte und Themenkreise zu Aufklärung, Anamnese und körperlicher Untersuchung Schwangerer, der Analgesie zur schmerzarmen vaginalen Entbindung, der Anästhesie zur Sectio caesarea, der postoperativen Analgesie nach Sectio caesarea, Besonderheiten in der postpartalen Phase, dem Umgang mit Risikokollektiven und Qualitätsindikatoren in der geburtshilflichen Analgesie und Anästhesie werden in diesem Kommentar dargestellt.
