Expertenkonsensus zur Prophylaxebehandlung des hereditären Angioödems

Zusammenfassung

Das hereditäre Angioödem, kurz HAE, ist eine chronische, genetische und stark beeinträchtigende Erkrankung mit intermittierend wiederkehrenden vaskulären Ödemen in mukosalem und submukosalem Gewebe oder in der Dermis und Subkutis. Diese Schwellungen beeinträchtigen das Wohlbefinden der Patienten sowohl körperlich als auch emotional stark. Je nach Lokalisation werden Patienten bei Ärzten unterschiedlicher Fachrichtungen vorstellig, nicht selten kommt es zu Fehldiagnosen wie Blinddarmentzündung oder Allergie und darauffolgenden Fehlbehandlungen. Eine HAE-Attacke kann auch akut lebensbedrohlich sein. Medikamente zur Behandlung der häufigeren Mastzell-vermittelten Angioödeme sind jedoch bei HAE unwirksam. Eine korrekte Diagnose der Erkrankung, die oft erstmals in Kindheit oder Jugend in Erscheinung tritt, ist daher essenziell für die effektive Behandlung. Herausfordernd sind hierbei besonders De-novo-Mutationen, bei denen die Familienanamnese negativ ist. Jedoch können verschiedene neue Behandlungsoptionen HAE-Patienten durch Prävention von Attacken und Minderung der Krankheitslast helfen. Wir fassen in diesem Review die Belastungen zusammen, denen Patienten mit HAE infolge ihrer Erkrankung selbst, aber auch infolge von Fehldiagnosen und -behandlungen ausgesetzt sind, sowie die Rolle der vorbeugenden Behandlung (Langzeitprophylaxe) für die Lebensqualität der Betroffenen und ihrer Familien. Darüber hinaus geben wir konkrete Hinweise, wie HAE früh erkannt werden kann, um Patienten frühestmöglich an Experten überweisen zu können. Unter Bezug auf die Empfehlungen der aktualisierten WAO/EAACI-Guideline (2022) plädieren wir für eine stärkere Rolle der Langzeitprophylaxe und die Förderung eines modernen, patientenzentrierten Managements des HAE mit Hilfe von "patient reported outcome measures" (PROMs) zur Kontrolle von Krankheitslast und Lebensqualität.
