Aortenstenose – welche diagnostischen Algorithmen und welche Therapie?

Zusammenfassung

Die Fortschritte der Kardiologie in den letzten 50 Jahren sind besonders gut anhand der Diagnostik und der Therapie der Aortenklappenstenose erkennbar. Während früher nach Klinik, EKG, Röntgenthorax und Herzkatheter mit transseptaler Technik bei jüngeren Patienten eine Operation in Betracht kam, ist die Diagnostik heute ganz auf die Echokardiographie ausgerichtet, ergänzt durch Bestimmung der Aortenklappenverkalkung in der Computertomographie (CT) bzw. erweiterter Diagnostik mittels Herz-MRT (Magnetresonanztomographie). Die Therapie der schweren Aortenklappenstenose ist nicht mehr nur mittels Aortenklappenersatz, sondern auch mithilfe der transluminalen Aortenklappenimplantation möglich. Imponierend bleibt die Verschiebung der Altersgruppen der behandelten Patienten. Unter dem 75. Lebensjahr wird zwar die Operation empfohlen, aber für ältere Menschen, besonders bei hohem Risiko, wird die interventionelle kathetergestützte Behandlung bevorzugt.
