Infiltration nach chirurgischer Knorpeltherapie

Gibt es eine Evidenz und welche Überlegungen stecken dahinter?

Zusammenfassung

Die leitliniengerechte chirurgische Knorpeltherapie bei fokalen Knorpelschäden bietet hocheffektive Möglichkeiten, Beschwerden bei Patienten nachhaltig zu mindern und die Entstehung einer frühzeitigen Arthrose zu verhindern oder zumindest zu verzögern. Am Kniegelenk hat sie das Potenzial, fast ein Viertel der durch Knorpelschäden entstehenden gelenkersatzpflichtigen Arthrosen zu vermeiden. Biologisch wirksame Injektionstherapien könnten diese Ergebnisse noch weiter verbessern. Auf Basis der aktuell verfügbaren Literatur und präklinischer Studien kann bezüglich intra- und postoperativ anwendbaren Injektionspräparaten ein positiver Effekt von plättchenreichem Plasma/Fibrin (PRP/PRF) und Hyaluronsäure (HA) auf die Knorpelregeneration und bei HA-Injektionen auch auf das klinische Outcome angenommen werden. Bezüglich fettgewebsbasierter Zelltherapie rechtfertigen die aktuellen wissenschaftlichen Daten noch keine Anwendungsempfehlung. Weitere Studien auch bezüglich Anwendungsintervallen, Anwendungszeitpunkten und Unterschieden bei verschiedenen Gelenken sind erforderlich.
