Weniger kann mehr sein – Beispiele zur Medikation bei älteren und geriatrischen Patienten aus aktuellen Studien

Zusammenfassung

Multimedikation, das heißt die Verordnung von 5 oder mehr unterschiedlichen Arzneimitteln zur medikamentösen Behandlung, ist ein typisches Problem älterer und geriatrischer Patienten. Sie prädisponiert zu unterschiedlichen negativen Gesundheitsfolgen, so etwa unerwünschten Arzneimittelwirkungen, Arzneimittelinteraktionen, potenziell inadäquater Medikation, reduzierten funktionellen Fähigkeiten, vermehrten Krankenhausaufenthalten und einer Erhöhung der Mortalität. Verschiedene Konsensgruppen und Fachgesellschaften haben Empfehlungen erstellt, wie Multimedikation bei geriatrischen Patienten gehandhabt werden kann. Wenngleich die Begrenzung von Multimedikation allgemein als notwendig erachtet wird, fehlt in vielen Bereichen noch Evidenz, Multimedikation bei geriatrischen Patienten zu begrenzen und Medikation zu reduzieren und abzusetzen. In diesem Beitrag werden anhand aktueller Beispielstudien zu potenziell inadäquater Medikation, Vitamin-D-Substitution und Psychopharmaka Handlungsfelder aufgezeigt, in denen eine kritische Prüfung der verordneten Medikation, eine kritische Prüfung von Indikationen sowie ein Absetzen von Wirkstoffen angezeigt erscheinen.
