Vibrationssyndrom der Finger

Eine Überarbeitung im Anerkennungsverfahren der BK 2104 ist nötig

Zusammenfassung

Physikalische Einwirkungen wie Kälte, Wärme, aber auch Vibrationen können Gesundheitsschäden verursachen, die im Rahmen einer beruflichen Exposition als Berufserkrankung anerkannt werden können. Seit 1979 ist in der Liste der Berufserkrankungen (BK) das vibrationsbedingte vasospastische Syndrom (BK 2104) verankert. Als wesentliche Hilfestellung in der Begutachtung von Berufserkrankungen existieren Merkblätter, die für Ärzte und Versicherungsinstitutionen die Grundlage für das Anerkennungsverfahren darstellen. Die BK 2104 besteht aus einer vaskulären und einer nervalen Komponente. Als funktionelle Durchblutungsstörung muss die vaskuläre Komponente der BK 2104 von organisch fixierten Durchblutungsstörungen wie dem Hypothenar-Hammer-Syndrom (HHS) oder der peripheren arteriellen Durchblutungsstörung abgegrenzt werden. Auch andere funktionelle Durchblutungsstörungen, die sich mit dem klinischen Bild eines primären oder sekundären Raynaud-Phänomens zeigen, müssen ausgeschlossen werden. Die nervale Komponente geht der Durchblutungsstörung meist Jahre voraus. Wichtige Differenzialdiagnosen sind das Karpaltunnelsyndrom und das Thoracic-outlet-Syndrom. Spezielle Untersuchungen wie die Pallästhesiometrie und die Thermometrie/Thermographie zeigen die typischen vibrationsbedingten Schädigungen. Die Funktionseinschränkung der Hand, welche die Minderung der Erwerbsfähigkeit im Wesentlichen definiert, kann mittels Fragebogen und neuropsychologischen Assessments abgeschätzt werden. Das aktuell vorliegende Merkblatt von 1979 muss aufgrund der wissenschaftlichen Erkenntnisse revidiert werden. Wesentliche Gesichtspunkte zu Pathophysiologie, Krankheitsbild sowie dem diagnostischen und differenzialdiagnostischen Vorgehen werden im Beitrag erläutert.
