Was ist gesichert in der Therapie der chronischen Veneninsuffizienz?

Zusammenfassung

Chronische Venenkrankheiten gehören zu den häufigsten Krankheiten. Sie können asymptomatisch sein, subjektive Symptome verursachen oder zu objektivierbaren Veränderungen wie Ödem, Hautveränderungen und Ulcus cruris führen. In letzterem Fall spricht man von einer chronischen Veneninsuffizienz (CVI). Die Varikose ist eine progrediente degenerative Erkrankung der Venenwand im oberflächlichen Venensystem der Beine, welche die Lebensqualität der Betroffenen entscheidend beeinträchtigen kann. Die Einteilung der chronischen Venenkrankheiten erfolgt mit der CEAP-Klassifikation nach klinischen, ätiologischen, anatomischen und pathophysiologischen Kriterien. Für die Beurteilung des Schweregrads dienen Instrumente wie der Venous Clinical Severity Score. Die Therapie chronischer Venenkrankheiten zielt auf die Besserung der subjektiven Beschwerden und objektivierbaren Veränderungen. Zudem sollen Komplikationen wie Venenentzündung und Ulkusentstehung vermieden werden. Invasive Verfahren, Kompressionstherapie und medikamentöse Therapie ergänzen sich; eine Kombination dieser Verfahren kann sinnvoll sein. Allgemeine physikalische Maßnahmen wie Hochlegen der Beine und häufiges Gehen gehören zu den Basismaßnahmen für jeden Patienten mit Venenkrankheit. In der nichtinvasiven Behandlung der symptomatischen Venenkrankheiten ist die Kompressionstherapie mit medizinischen Kompressionsstrümpfen der Goldstandard, eventuell ergänzt durch venenaktive Medikamente. Eine Varikose sollte möglichst ausgeschaltet werden. Strippingoperation und die weniger invasiven endovenösen thermischen Verfahren zeigen vergleichbare Ergebnisse bei der Stammvarikose. Schaumverödung und perkutane Phlebextraktion sind die Methoden der Wahl zur Ausschaltung der Astvarikose. Varizenrezidive bzw. ein Fortschreiten des Krankheitsbildes sind jedoch häufig.
