Rekurrierende Fiebersyndrome – Subgruppe der autoinflammatorischen Erkrankungen

Zusammenfassung

Rekurrierende Fiebersyndrome werden pathophysiologisch zu den autoinflammatorischen Erkrankungen (AID) gezählt. Die klassischen seltenen monogenen Interleukin‑1 vermittelten AID (z. B. familiäres Mittelmeerfieber, TNF-Rezeptor-1-assoziiertes periodisches Syndrom, Cryopyrin-assoziiertes periodisches Syndrom, Hyper-IgD-Syndrom/Mevalonatkinasedefizienz), die Defizienz der Adenosindesaminase 2, aber auch die polygenetischen AID (z. B. das PFAPA-Syndrom: periodisches Fieber, aphthöse Stomatitis, Pharyngitis und zervikale Adenitis) sind durch rezidivierende, selbstlimitierende, systemische Entzündungsschübe gekennzeichnet und gehen mit einer Dysregulation des angeborenen Immunsystems einher.
Die Gemeinsamkeiten der Erkrankungen liegen neben den variabel ausgeprägten Fieberepisoden in den typischen Organbeteiligungen (v. a. Haut, Gelenke, Gastrointestinaltrakt). Ethnische Herkunft, Fieberkalender, erhöhte Inflammationsparameter und krankheitsassoziierte Genmutationen sind diagnostisch wegweisend. Fehlende funktionelle laborchemische Untersuchungen führen jedoch auch heute noch zu einer erheblichen Diagnoseverzögerung. Moderne molekulargenetische Untersuchungen können Diagnosen bestätigen, neue Entitäten detektieren sowie dazu beitragen, die Definition und die Pathophysiologie der AID weiterzuentwickeln.
