Zusammenfassung

In der Herz-Kreislauf-Medizin als einem der umfangreichsten Leistungsbereiche des Gesundheitswesens lassen sich zahlreiche Beispiele für Überdiagnostik und Übertherapie finden. Der vorliegende Beitrag greift drei davon heraus, eines aus der Kardiologie und zwei aus der Gefäßmedizin. Beim Thema der chronischen koronaren Herzkrankheit geht es um den verlässlichen Ischämienachweis vor koronaren Interventionen. In den Abschnitten über die Duplexsonographie der Halsgefäße und die Thrombophiliediagnostik wird erörtert, welche validen klinischen Fragestellungen überhaupt die Durchführung dieser Untersuchungen rechtfertigen. Das Schadenspotenzial der Überdiagnostik liegt in diesen beiden Fällen sowohl beim unnötigen Ressourcenverbrauch als auch – viel bedeutsamer – in der sich anschließenden Übertherapie. Die wenigen angemessenen Indikationen werden konkret beschrieben.
