Zusammenfassung

Seit Jahrzehnten ist der Hämoglobin A1c(HbA1c)-Wert als Parameter für die regelmäßige Glukosestoffwechselkontrolle bei Diabetes mellitus etabliert. Während der HbA1c-Wert die Glukosekonzentrationen der letzten 2 bis 3 Monate widerspiegelt, sind seit einiger Zeit aus der kontinuierlichen Glukosemessung (CGM) abgeleitete Parameter, die einen kürzeren Zeitraum abbilden können, verfügbar. Es wird sogar diskutiert, den HbA1c-Wert durch diese Parameter zu ersetzen. Die Zeit im Zielbereich und die Zeiten oberhalb und unterhalb des Zielbereiches geben Aufschluss darüber, wie lange Glukosewerte im bzw. über und unter dem Zielbereich lagen. Diese Parameter können bei CGM-Anwendern erhoben werden; die Berechnung erfolgt in der Regel in den herstellerspezifischen CGM-Softwares. Für die Beurteilung der Parameter sollten jedoch die Compliance der Anwender sowie die (mess)technischen Aspekte und Qualitätsanforderungen der CGM-Systeme beachtet werden. Aktuell wird empfohlen, CGM-abgeleitete Parameter als Ergänzung zum HbA1c-Wert im Rahmen der Diabetestherapie anzusehen.
