Pathologie der Mamillenregion

I. Morbus Paget der Mamille, Varianten und Differenzialdiagnosen

Zusammenfassung

Der klassische M. Paget der Mamille ist histologisch charakterisiert durch eine intraepidermale Tumorinfiltration, die von einem intraduktalen oder invasiven Mammakarzinom ausgeht, immunhistologisch durch eine regelhafte Überexpression von HER2 und klinisch durch ekzemartige Veränderungen von Mamille und Areola. Zu den Varianten mit davon abweichenden histologischen, immunhistologischen und klinischen Erscheinungsformen gehören der isolierte M. Paget der Mamille, der anaplastische M. Paget, der M. Paget mit Invasion und der pigmentierte M. Paget der Mamille. Differenzialdiagnostisch ist der M. Paget der Mamille abzugrenzen gegenüber benignen Veränderungen, insbesondere der Toker-Zell-Hyperplasie, dem Mamillenekzem und seltenen Dermatosen.

Unter den Erkrankungen der Mamma nehmen die Veränderungen der Mamillenregion mit ihren anatomischen Besonderheiten eine Sonderstellung ein [57]. Klinisch stellt sich nicht selten die Differenzialdiagnose einer entzündlichen gegenüber einer präneoplastischen oder neoplastischen Veränderung und führt zur Biopsie. Vonseiten der Pathologie weist die Mamille neben den klassischen Läsionen wie dem M. Paget der Mamille oder dem Mamillenadenom auch eine Reihe von seltenen und außergewöhnlichen Befunden auf. In diesem Review diskutieren wir aus aktueller Sicht die Veränderungen der Mamille, die den M. Paget und seine Differenzialdiagnosen betreffen.

Klassischer M. Paget der Mamille

Der mammäre M. Paget ist histologisch durch die Infiltration der Epidermis der Mamillenhaut mit glandulär differenzierten Tumorzellen charakterisiert [29]. Zu den klinischen Befunden gehören ekzematöse, nässende oder auch trockene bzw. schuppende Veränderungen der Mamille [52]. Es gilt die Regel, dass chronische ekzematöse Veränderungen zum Ausschluss eines M. Paget histologisch abgeklärt werden sollen [20]. Dem M. Paget der Mamille liegt in der Mehrzahl der Fälle ein invasives Mammakarzinom (53,4 %) oder ein rein intraduktales Karzinom (DCIS, 32,9 %) zugrunde, während der isolierte M. Paget der Mamille eine seltenere Form (13,7 %) darstellt. Bei den zitierten relativen Häufigkeiten handelt es sich um eine Erhebung der US-amerikanischen epidemiologischen SEER-Datenbank [64]. Für Deutschland liegen keine epidemiologischen Daten vor. Bezogen auf alle invasiven und intraduktalen Mammakarzinome beträgt die Häufigkeit des M. Paget etwa 1 %. Das mediane Patientenalter bei M. Paget mit assoziiertem Mammakarzinom beträgt 60 Jahre und unterscheidet sich dadurch nicht signifikant vom Alter beim Mammakarzinom ohne M. Paget [6]. Beschrieben ist der M. Paget der Mamille auch beim Mann, wobei hier die Abgrenzung gegenüber ekzematösen Veränderungen, aber auch dem plattenepithelialen Carcinoma in situ (M. Bowen) und dem malignen Melanom wichtig ist [1].

Histologie und Immunhistologie

Histologisch zeigen klassische Fälle eines M. Paget eine nestartige und einzelzellige Ausbreitung von Tumorzellen innerhalb der Epidermis, die in den meisten Fällen ein breites und feingranuläres Zytoplasma und große, zentral gelegene Zellkerne mit wechselnd stark ausgeprägten Kernatypien sowie häufig prominente Nukleolen besitzen (Abb. 1a). Seltener sind tubuläre oder glanduläre Differenzierungen [54] oder auch siegelringzellige oder spindelzellige Tumorzellen [34]. Typischerweise liegen die Paget-Zellen in den unteren epidermalen Schichten, auch entlang der Haarfollikel, steigen in die oberen Schichten der Epidermis auf und schilfern mit dem Stratum corneum ab [22]. Dermale entzündliche Veränderungen in Form eines Ödems, granulozytenreicher Entzündungszellinfiltrate und Kapillarproliferationen können mit dem M. Paget assoziiert sein. Der seltene M. Paget der männlichen Mamma weist histologisch ähnliche Charakteristika auf.
Immunhistologisch ist der M. Paget der Mamille (Abb. 1a) im Unterschied zum extramammären M. Paget in über 90 % der Fälle durch eine Überexpression von HER2 (Abb. 1b) charakterisiert [15, 37], häufig ist auch eine Überexpression des Androgenrezeptors (Abb. 1c). Seltener ist ein HER2-/ER+-Immunphänotyp [3]. Paget-Zellen sind fast ausnahmslos positiv für Zytokeratin 7 (CK7; Abb. 1d). Nur vereinzelt sind auch CK7-negative Fälle beschrieben, z. B. in Zusammenhang mit einem solid-papillären Karzinom [46] oder isolierten M. Paget [41]. Die Angaben über die Häufigkeit der Expression des Östrogenrezeptors schwanken in der Literatur zwischen 11 % [33] und 44 % [7].

Isolierter M. Paget der Mamille

In klinischen Fallserien ist der M. Paget der Mamille in 92–100 % mit einem duktalen Carcinoma in situ (DCIS) oder einem invasiven Mammakarzinom assoziiert, die als Ausgangspunkt des M. Paget angesehen werden [25, 26, 62]. Dagegen kann bei der seltenen, isolierten Form des M. Paget der Mamille kein DCIS oder ein Mammakarzinom nachgewiesen werden. Es stellt sich daher die Frage, ob es sich bei dem isolierten M. Paget möglicherweise um eine pathogenetisch und tumorbiologisch unterschiedliche Form des M. Paget der Mamille handelt. Hinweise dafür in der Literatur betreffen die unterschiedliche klinische Präsentation mit einem ausgedehnten und teilweise die Areola überschreitenden Befund, einer makroskopisch unscharfen Begrenzung und einer längeren, nicht selten jahrelangen Anamnese bis zur Diagnosestellung sowie einem prognostisch günstigeren klinischen Verlauf [11]. Aufgrund der Ähnlichkeit mancher Fälle eines isolierten M. Paget der Mamille mit einem extramammären M. Paget wurde der Begriff „M. Paget der Mamille vom extramammären Typ“ vorgeschlagen [12, 44]. Wie bei dem M. Paget der Vulva kann in solchen Fällen eine topische Behandlung mit Imiquimod® wirksam sein [38].

Sonderformen des mammären M. Paget

Die in der dermatologischen Literatur beschriebenen Varianten des anaplastischen M. Paget (Abb. 2a) und des akantholytisch-anaplastischen M. Paget der Mamille zeigen eine bowenoide Morphologie mit diffuser neoplastischer Infiltration der Epidermis und reduzierter oder fehlender Muzinbildung. Diese Formen des M. Paget stellen eine undifferenzierte Variante des klassischen M. Paget dar und sind PAS-negativ, negativ für GCDFP-15, jedoch typischerweise positiv für CK7 und HER2 [39, 45]. Ein ähnliches Bild mit undifferenzierten intraepidermalen Tumorzellen und mit einer Verringerung muzinpositiver Paget-Zellen kann beim klassischen M. Paget mit starken sekundär entzündlichen Veränderungen auftreten.
Bei dem ebenfalls seltenen invasiven M. Paget (Abb. 2c–e) handelt es sich um einen M. Paget der Mamille mit direkter Tumorinvasion (meist Mikroinvasion) der Dermis, jedoch ohne Assoziation mit bzw. Abseits von einem zugrunde liegenden invasiven Mammakarzinoms [10]. Auch diese Fälle sind in der Regel HER2-positiv. Die Häufigkeit des invasiven M. Paget wird mit 7,8 % angegeben [32]. Lymphknotenmetastasen oder isolierte Tumorzellen in axillären Lymphknoten wurden beschrieben [42, 48, 49], jedoch keine Fälle von Fernmetastasen bei Fehlen eines assoziierten Mammakarzinoms [32].
Der pigmentierte M. Paget (Abb. 3a–c) stellt eine seltene Variante eines im Übrigen histologisch und immunhistologisch typischen M. Paget dar [55]. Klinisch liegt eine flache, pigmentierte makula- oder auch plaqueartige pigmentierte Läsion vor. Histologisch handelt es sich um intraläsionale pigmentierte dendritische Zellen oder Melanozyten, die positiv für melanozytäre Marker (S100, SOX-10, Melan‑A, HMB45) sind, bei typischer CK7-Positivität der Paget-Zellen. In vielen Fällen liegt ein isolierter pigmentierter M. Paget der Mamille oder der Areola vor, aber auch pigmentierte Formen des M. Paget mit einem assoziierten intraduktalen oder invasiven Karzinom sind beschrieben. Der pigmentierte M. Paget kommt in morphologisch ähnlicher Form extramammär vor [43] (auch an ungewöhnlichen Lokalisationen wie dem Oberschenkel oder der Axilla [8, 18]). Immunhistologisch lässt sich der pigmentierte M. Paget gegenüber einem Melanoma in situ unschwer durch die Expression von Zytokeratinen abgrenzen [2, 16].

Differenzialdiagnosen des M. Paget

Die Differenzialdiagnose großer, heller Zellen innerhalb der Epidermis der Mamille umfasst Paget-Zellen, Toker-Zellen, epidermale Klarzellen oder melanozytäre Zellen. Davon ausgehend ist die wichtigste Differenzialdiagnose des M. Paget die Abgrenzung gegenüber benignen Befunden wie dem Mamillenekzem oder seltenen Dermatosen, während das maligne Melanom der Mamille oder anderweitige prämaligne oder maligne Läsionen Raritäten darstellen.

Toker-Zell-Hyperplasie

Toker-Zellen sind klarzellige, CK7-positive Zellen der Epidermis der Mamille [13, 58], die mutmaßlich ihren Ursprung in den Milchgängen haben (Abb. 4a; [61]) oder auch mit Talgdrüsen assoziiert sind [47]. Neben den Zytokeratinen (CK7 [Abb. 4b], AE-1 + 3, CAM‑5.2) und epithelialem Membranantigen (EMA) exprimieren Toker-Zellen auch Östrogen- und Progesteronrezeptoren (ER, PR), sind jedoch in der Regel negativ für HER2 (Abb. 4c; [9]). Aufgrund des Immunprofils werden Toker-Zellen als möglicher Ausgangspunkt des isolierten M. Paget der Mamille angesehen [35]. Die Toker-Zell-Hyperplasie ist eine benigne Läsion der Mamille mit Auftreten basaler, nestartiger und pagetoid aufsteigender Toker-Zellen intraepidermal, jedoch einer im Gegensatz zum M. Paget blanden Morphologie der Klarzellen [9].

Mamillenekzem

Das Mamillenekzem kann sich im Rahmen einer Neurodermitis oder als isolierte Läsion der Mamille manifestieren, wobei in beiden Konstellationen histologisch ähnliche Befunde vorliegen, das Immuninfiltrat betreffend [56]. Betroffen sind überwiegend weibliche Patienten und beim Mamillenekzem mit atopischer Dermatitis junge Patienten mit einem medianen Erkrankungsalter von 19 Jahren sowie meist bilateralem Befund gegenüber einer unilateralen Lokalisation des isolierten Mamillenekzems ohne Atopie [21, 59]. Histologisch findet man in der Biopsie typischerweise eine blockartige Akanthose mit leichter Parakeratose, Spongiose und perivaskulärem lymphohistiozytärem Infiltrat mit vereinzelten Eosinophilen im oberen Korium. Die Veränderungen können einem Klarzellakanthom der Mamille ähneln [28].

Seltene Dermatosen

Das Auftreten von epidermalen Klarzellen der Mamille in Zusammenhang mit einer dyskeratotischen Verhornung wird als pagetoide Dyskeratose [14] oder, in Verbindung mit einer scharf begrenzten Akanthose, auch als Klarzellakanthom der Mamille [23] bezeichnet. Die hierbei auftretenden epidermalen Klarzellen sind negativ für CK7 und entsprechen Keratinozyten mit optisch leeren zytoplasmatischen Vakuolen, welche am ehesten auf Fixationsartefakte zurückzuführen sind. Bei der nävoiden Hyperkeratose der Mamille und Areola handelt es sich um eine langsam progrediente verruköse Läsion mit brauner Pigmentierung der Mamille und Areola [36]. Histologisch findet man eine breite Akanthose mit Hyperkeratose und eingeschlossene Hornperlen [27]. Zur Differenzialdiagnose s. Tab. 1.

Weitere Differenzialdiagnosen

Seltene Differenzialdiagnosen von Läsionen, die mit intraepidermalen Klarzellen einhergehen, betreffen die klarzellige Papulose [63], dermale Adnextumoren mit Klarzellen wie das ekkrine Porom oder das klarzellige Hidroadenom [19], die Langerhans-Zell-Histiozytose [4], den pagetoiden M. Bowen [60] und das maligne Melanoma in situ. Die verschiedenen intraepidermale Klarzellen unterscheiden sich durch ihr Immunprofil (Tab. 2), welches für die Differenzialdiagnose der infrage kommenden Läsionen hilfreich ist [30].

Fazit für die Praxis

  • Die Diagnose eines klassischen M. Paget der Mamille stützt sich auf das charakteristische Immunprofil mit zumeist nachweisbarer HER2-Überexpression und auf die Assoziation mit einem duktalen Carcinoma in situ.
  • Zu den selteneren Varianten des M. Paget der Mamille gehören der pigmentierte, der anaplastische und der invasive M. Paget der Mamille sowie Formen mit nichtklassischem Immunprofil wie Negativität von HER2 oder Zytokeratin 7 (CK7).
  • Der isolierte M. Paget der Mamille ohne Nachweis eines zugrunde liegenden Mammakarzinoms nimmt eine Sonderstellung ein und kann klinische und histologische Charakteristika aufweisen, die für den extramammären M. Paget typisch sind.
  • Die Toker-Zell-Hyperplasie ist eine seltene gutartige Läsion der Mamille mit intraepidermaler Infiltration blander, CK7-positiver Epithelien und muss gegenüber dem M. Paget abgegrenzt werden.
  • Zu den Dermatosen, die mit intraepidermalen Klarzellen, Akanthose, Hyperkeratose und Pigmentierung der Mamillenepidermis einhergehen, gehören die pagetoide Dyskeratose, das Klarzellakanthom der Mamille und die nävoide Hyperkeratose der Mamille und Areola.

Bei Immuntherapien das erhöhte Thromboserisiko beachten

Unter modernen Systemtherapien versechsfacht sich das VTE-Risiko. Warum diese Daten relevant für die Behandlung krebsassoziierter Thrombosen sind, erläutert Prof. F. Langer im Interview. So kann es durch Immuntherapien zu inflammatorischen Syndromen z.B. im GI-Trakt kommen. Nebenwirkungen wie Durchfall oder Mukositis haben dann Einfluss auf die Wirksamkeit oraler Antikoagulantien. Aber auch in punkto Blutungsrisiko ist Vorsicht geboten. Wann hier bevorzugt NMH eingesetzt werden sollten, erläutert Prof. Langer im Interview.

CAT-Management ist ganz einfach – oder doch nicht?

Krebsassoziierte venöse Thromboembolien (CAT) haben in den vergangenen Jahren stetig zugenommen. Was hat der Anstieg mit modernen Antitumortherapien zu tun? Venöse Thromboembolien sind relevante Morbiditäts- und Mortalitätsfaktoren in der Onkologie. Besonders hoch sind die Risiken bei Tumoren des Abdominalraums.
