Update der S3-Leitline Reizdarmsyndrom

Zusammenfassung

Meteorismus, Bauchschmerzen und Veränderungen des Stuhlgangs sind häufige Beratungsanlässe in der Hausarztpraxis. Auslöser hierfür kann ein Reizdarmsyndrom (RDS) sein, ein heterogener, gastrointestinaler Symptomkomplex bisher ungeklärter Ursache. Die sorgfältige Anamnese und gezielte Stufendiagnostik bilden die Basis der hausärztlichen Abklärung. Beim diarrhödominanten RDS oder bei Mischformen liegt häufig eine behandelbare Differenzialdiagnose vor. Aufgrund der Heterogenität der Erkrankung gibt es keine Standardtherapie. Die Grundlage einer RDS-Behandlung bilden die Patient:innenedukation und die Etablierung eines biopsychosozialen Erkrankungsmodells. Die Ernährung orientiert sich an individuellen Symptomtriggern. Die Evidenzlage für Probiotika ist positiv. Medikamentöse Therapien erfolgen nach Leitsymptom. Multimodale Therapiekonzepte haben eine bessere Wirksamkeit als Monotherapien und sollen gemeinsam mit den Patient:innen und vernetzt mit Spezialist:innen umgesetzt werden.
