Endoskopische Diagnostik und Therapieoptionen beim Gallengangskarzinom

Zusammenfassung

Die Diagnostik beim Gallengangskarzinom konzentriert sich auf die Differenzialdiagnose bei Gallengangsstrikturen sowie die präoperative Diagnostik, die endoskopische Therapie auf eine palliative oder präoperative Ableitung. Die diagnostischen und therapeutischen Konzepte in der Gallengangsendoskopie sind entscheidend vom operativen oder nichtoperativen Management dieser Tumoren abhängig. Bei der primären Diagnostik werden in der Regel bildgebende Verfahren wie Sonographie, CT, ERCP und MRCP eingesetzt. Die Notwendigkeit einer Gewebediagnostik vor einer onkologischen Therapie ist bei den Gallengangstumoren umstritten, die Zytologie kann jedoch zur Risikostratifizierung herangezogen werden. Kontrovers wird nach wie vor der Nutzen oder Schaden einer präoperativen Stenteinlage bei pankreatobiliären Tumoren diskutiert. Zusammenfassend kann gesagt werden, dass die Standardtherapie in der Palliation hilärer Gallengangstumoren immer noch das bildgebungsgesteuerte (Sono oder MR) einseitige Stenting ist. Andere Verfahren wie Metallstenteinlage, Brachytherapie oder PDT sind Einzelfällen vorbehalten, wobei die beste Evidenz noch für die PDT besteht.
