Evidenz in der minimal-invasiven onkologischen Chirurgie der Leber

Zusammenfassung

Hintergrund

In der Literatur steigt die Anzahl der Publikationen zur minimal-invasiven (MIC) Leberchirurgie und mit ihr die Evidenz für das Verfahren. Trotzdem scheint dieses Verfahren noch keine breite Anwendung zu finden.

Ziel der Arbeit

Analyse und Darstellung der aktuellen Literaturlage zur MIC-Leberchirurgie.

Material und Methoden

Strukturierte PubMed-Literatursuche und Literaturanalyse.

Ergebnisse

Die Literatur beschreibt eine onkologische Gleichwertigkeit der minimal-invasiven mit der offenen Leberchirurgie, wobei neben atypischen prinzipiell auch Major-Resektionen (Hemihepatektomien und Sektorektomien), zweizeitige Konzepte und Reresektionen technisch möglich sind. Die MIC-Leberchirurgie ist mit einem geringeren Blutverlust und anderen immunologischen Vorteilen verbunden und bedarf einer kürzeren Krankenhausverweildauer. Die höchste Evidenz besteht für Resektionen bei kolorektalen Lebermetasten (KRLM), für die ein systematischer Review sogar langfristig einen Überlebensvorteil aufgezeigt hat. Die laparoskopische und robotische Leberchirurgie können aus onkologischer Sicht aktuell als gleichwertig angesehen werden.

Diskussion

Gemäß der aktuellen Literatur ist die MIC-Leberchirurgie für die Resektion von KRLM und hepatozellulären Karzinomen empfehlenswert.
