Genetisches Risiko bei metabolischer Fettlebererkrankung

Gemeinsame Risikofaktoren für Fettlebererkrankung, Diabetes und Hyperlipidämie

Zusammenfassung

Die nichtalkoholische Fettlebererkrankung (NAFLD) ist eng mit dem metabolischen Syndrom vergesellschaftet und soll laut Expertenempfehlung in metabolisch assoziierte Fettlebererkrankung (MAFLD) umbenannt werden. Diese stellt mit einer Prävalenz von 25 % für die Gesamtbevölkerung und von über 50 % bei Patienten mit Diabetes mellitus Typ 2 (T2DM) die häufigste Lebererkrankung in Europa dar. Mittels genetischer Assoziationsstudien wurde in den letzten Jahren eine Reihe von Genvarianten identifiziert, die die Entwicklung und Progression der MAFLD maßgeblich beeinflussen. Es wird angenommen, dass ungefähr 50 % der beobachteten interindividuellen Schwankungen des Leberfettgehalts durch genetische Faktoren bestimmt werden. Besondere Bedeutung kommt den Risikovarianten in den Genen PNPLA3 („patatin like phospholipase-containing 3“), TM6SF2 („transmembrane 6 superfamily member 2“), MBOAT7 („membrane bound O‑acyltransferase domain containing 7“), GCKR (Glukokinaseregulator) und HSD17B13 (Hydroxysteroid 17-Betadehydrogenase 13) zu. Das durch diese Varianten vermittelte Risiko für eine Steatose, Steatohepatitis, Leberzirrhose und hepatozelluläres Karzinom wirkt additiv. Ob die Bestimmung dieser Genvarianten künftig im klinischen Alltag zur Risikostratifizierung oder zur Vorhersage des Ansprechens auf mögliche therapeutische Interventionen eingesetzt werden kann, muss in klinischen Studien überprüft werden. Im Folgenden werden die pathophysiologischen Auswirkungen der Risikogenvarianten auch im Bezug zu weiteren Aspekten des metabolischen Syndroms (T2DM, Hyperlipidämie) diskutiert. Der Effekt der Risikogenvarianten auf den Metabolismus nimmt mit steigendem Körpergewicht zu und unterstreicht die wechselseitige Beeinflussung von MAFLD, T2DM, Hyperlipidämie und Adipositas.
