Zusammenfassung

Die Ergebnisse experimenteller und epidemiologischer Studien deuten darauf hin, dass Diabetes die Risiken für bestimmte gastrointestinale und gynäkologische Tumoren erhöht. Am besten belegt ist dies hinsichtlich des Typ-2-Diabetes, wobei ähnliche Zusammenhänge für Prädiabetes, Gestationsdiabetes und Typ-1-Diabetes beschrieben sind. Mechanistisch ist dies auf tumorfördernde Eigenschaften einer hochregulierten Insulin-IGF-1-Achse (IGF: „insulin-like growth factor“) und auf Hyperglykämien zurückzuführen. Zudem tragen adipositasassoziierte Stoffwechselveränderungen wesentlich zum Krebsrisiko bei. Daten aus Beobachtungsstudien, die niedrigere Krebsrisiken unter Metformin- vs. Insulintherapie suggerieren, sind mit Vorsicht zu interpretieren. Derzeit wird der tertiärpräventive Nutzen von Metformin unter Krebspatienten mit und ohne Diabetes untersucht. Ansonsten dienen Lebensstilinterventionen und eine gute medikamentöse Einstellung in frühen Phasen der Diabetesentstehung auch der Krebsprävention.
