Zusammenfassung

Über die beste Operationstechnik zur Versorgung von Narbenhernien liegt eine Reihe von Publikationen vor mit Erfahrungsberichten auf der einen und randomisierten klinischen Studien auf der anderen Seite der Evidenzskala. Der ultimative Beweis für die beste Operationstechnik ist bisher jedoch nicht erbracht worden. In kaum einem anderen Bereich der Chirurgie sind die Variabilität und die daraus resultierenden potenziellen Therapieziele so groß. Aufgabe des Chirurgen ist es, dem Patienten aus holistischer Sicht die optimale Empfehlung aus dem „Katolog“ der Möglichkeiten zu geben. Der Beitrag beschreibt die Operationstechniken für Netze zur Verstärkung (in Kombination mit einer anatomiegerechten Rekonstruktion) und zum Ersatz der Bauchdecke (mit Überbrückung bzw. Bridging des Defektes).

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Prof. Dr. med. Gregor Antoniadis Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
