Vaginale Lasertherapie – Mythen und Fakten

Zusammenfassung

In der aktuellen AWMF-Leitlinie (Arbeitsgemeinschaft Medizinisch-Wissenschaftlicher Fachgesellschaften) „Harninkontinenz der Frau“ (S2k) wird eine offene Empfehlung zur optionalen vaginalen Lasertherapie bei Patientinnen mit leichter und mittelschwerer Belastungsinkontinenz ausgesprochen. Diese Empfehlung findet in der EAU-Leitlinie (European Association of Urology) derzeit keine Entsprechung und wird auf ihre Evidenz geprüft. Die beiden zur Verfügung stehenden Lasersysteme (Erbium:YAG und CO2-Laser) sind – bei limitierter Datenlage – äquieffektiv in der Behandlung leicht belastungsinkontinenter Frauen. Probleme bei der Vergleichbarkeit von Studien bzgl. Inkontinenzdefinitionen, Schweregradeinteilungen, Erfolgsmessungen und Berücksichtigung unterschiedlicher Ätiologien werden beleuchtet. Nach Sichtung der Prognostikatorforschung im Bereich der vaginalen Lasertherapie wird ein Profil der idealen Laserkandidatin zur erfolgsorientierten Patientenselektion entwickelt, welches junges Alter, einen normalen Body Mass Index, einen guten Östrogenisierungsstatus, eine reine Belastungsinkontinenz aufgrund einer urethralen Hypermobilität und einen Urinverlust im 1‑h-Vorlagentest von < 8 g umfasst.
