Kognitive Dysfunktion bei Herzinsuffizienz – Ursachen und Folgen

Zusammenfassung

Etwa 40 % der Patienten mit Herzinsuffizienz zeigen kognitive Defizite wie Konzentrationsschwierigkeiten, Aufmerksamkeitsstörungen und Gedächtnisschwäche. Betroffene Patienten werden häufiger hospitalisiert und versterben früher. Dies könnte durch eine geringere Adhärenz, verursacht durch mangelndes Krankheitswissen und fehlende Handlungsstrategien, bedingt sein. Die Pathogenese kognitiver Leistungseinbußen bei Herzinsuffizienz ist multifaktoriell. Dabei spielen sowohl die Herzinsuffizienz selbst als auch kardiovaskuläre Risikofaktoren und Komorbiditäten eine Rolle. Orientierende neuropsychologische Untersuchungen können einen ersten Hinweis auf das Vorliegen kognitiver Defizite geben. Wenn diese identifiziert sind, können Ursachen gezielt behandelt und die Versorgung durch bestimmte Maßnahmen, wie etwa die Einbindung von Bezugspersonen, optimiert werden.
