Diagnose und Therapie der idiopathischen intrakraniellen Hypertension

Die idiopathische intrakranielle Hypertension (IIH) ist durch eine intrakranielle Liquordruckerhöhung mit den Leitsymptomen Kopfschmerzen und durch ein Papillenödem hervorgerrufene Sehstörungen charakterisiert. Sie zählt zu den seltenen Erkrankungen, die jedoch aufgrund einer Zunahme der Risikofaktoren in der Gesellschaft an Bedeutung gewinnt. Die IIH tritt typischerweise bei übergewichtigen Frauen im gebärfähigen Alter auf. Die Pathogenese ist nicht endgültig geklärt. Die Symptome sind gleichzeitig suspekt für eine sekundäre intrakranielle Hypertension, weshalb die Diagnose erst nach Ausschluss mittels Bildgebung und anschließender Lumbalpunktion gestellt werden kann. Therapeutisch wird ein Stufenschema aus Gewichtsreduktion, medikamentöser Therapie, wiederholten Lumbalpunktionen und operativen Verfahren mit dem Ziel der Senkung des Liquordrucks empfohlen.
