39. Hydrozephalus

Zusammenfassung

Als Hydrozephalus wird aus pathologisch-anatomischer Sicht eine Erweiterung der inneren und/oder äußeren Liquorräume bezeichnet. Falls eine Liquorzirkulationsstörung als Ursache zugrunde liegt, geht der Hydrozephalus meist mit einer intrakraniellen Drucksteigerung einher. Eine Liquorzirkulationsstörung stellt ein Missverhältnis zwischen Produktion und Abfluss bzw. Resorption des Liquors dar. In der überwiegenden Zahl der Fälle ist der Abfluss – sei es durch eine Passagestörung oder durch eine Resorptionsstörung – behindert. Eine echte Überproduktion ist nur in Fällen eines Plexustumors (Plexuspapillom/-karzinom) bekannt. Dem Hydrozephalus liegt ein multifaktorieller Pathomechanismus zugrunde, er bildet eine gemeinsame Endstrecke zahlreicher, heterogener Erkrankungen des zentralen Nervensystems. In der Intensivmedizin spielt nur der Hydrozephalus mit einer akuten oder chronischen intrakraniellen Druckerhöhung eine Rolle.
