Zusammenfassung

Die Therapie mit Lokalanästhetika (TLA) findet bis heute breiten Einsatz bei vielen banalen Alltagsbeschwerden. Das Grundlagenwissen über die pleiotropen molekularen Wirkungen von Lokalanästhetika ist umfangreich. Kontrollierte klinische Studien gibt es jedoch nur wenige. Die Indikationen lassen sich fünf Bereichen zuordnen: chronische Schmerzen, chronische Entzündungen, funktionelle Beschwerden, vegetative Beschwerden und Durchblutungsstörungen. Wichtige Kontraindikationen sind Allergien gegen Lokalanästhetika und Nadelangst. Eine häufige, aber harmlose Nebenwirkung ist die Kreislaufschwäche unmittelbar nach Behandlung. Schwere Nebenwirkungen sind selten. In der Gynäkologie findet die TLA Einsatz bei Vulvodynie, chronischer Zystitis und Kolpitis, Dysmenorrhö, unklaren Unterbauchbeschwerden, zyklischen Beschwerden und Narbenschmerzen. Indikationen in der Endokrinologie sind therapieresistente Schmerzen bei Endometriose und klimakterische Beschwerden bei Kontraindikation von Hormonen.
