Zusammenfassung

Das Spektrum der chirurgischen Verfahren zur minimal-invasiven Behandlung der benignen Prostatavergrößerung (BPH) hat sich über die letzten 2 Jahrzehnte signifikant verbreitert. Die einfache suprapubische Prostatektomie („simple prostatectomy“/„subtotal prostatectomy“, SP) hat gravierend an Bedeutung verloren. Die transurethrale Resektion der Prostata (TURP) wurde weiter standardisiert und durch die Einführung der bipolaren Technik sowie Niederdrucksysteme sicherer gemacht.
Die transurethralen (endoskopischen) Enukleationstechniken (endoskopische Enukleation der Prostata, EEP) stellen zunehmend eine wichtige Konkurrenz, insbesondere bei größeren Adenomvolumina, zu der als Goldstandard geltenden TURP dar und lösen die SP ab. Dieser Vorgang ist v. a. durch die rasante Entwicklung der Lasertechnik bedingt, welche, ähnlich wie in der Steintherapie, das Gesicht der modernen BPH-Behandlung nachhaltig verändert. Diese Änderung führte zur Beschreibung, klinischen Erprobung und Standardisierung zahlreicher Operationsmethoden, welche in diesem Artikel systematisch beschrieben werden. Zudem wurden Bestrebungen unternommen, weitere Energiequellen, wie z. B. den bipolaren Strom, in der EEP zu nutzen. Hinsichtlich der wissenschaftlichen Objektivierbarkeit werden regelmäßig hochwertige Studien publiziert, welche die Position der EEP weiter stärken.
