Zusammenfassung

Das Ovarialkarzinom ist die achthäufigste Krebserkrankung der Frau in Deutschland und aufgrund der häufig späten Diagnosestellung mit hoher Mortalität verbunden. Die Erkrankung ist in Abhängigkeit von der Tumorbiologie von hoher Heterogenität in Bezug auf Entstehung, Therapie und Prognose geprägt. Eine sinnvolle Früherkennungsmaßnahme existiert nicht. Die Prognose ist bestimmt durch einige Faktoren, von denen die qualitätsgesicherte Therapie als beinflussbar und das Tumorstadium bei Diagnosestellung als nicht beinflussbar herausstechen. In der Behandlung der Erkrankung kommt insbesondere der stadiengerechten Operation und der anschließenden Systemtherapie Bedeutung zu. Die operative Therapie ist im fortgeschrittenen Stadium von erheblichem Ausmaß geprägt. Das Outcome der Patientinnen lässt sich hier durch moderne perioperative Verfahren verbessern. Ein Tumorrezidiv stellt abhängig vom Ausmaß nicht mehr per se eine reine Palliativsituation dar. Hier sind Rezidivoperation, Chemotherapie in Kombination mit dem Neoangiogenesehemmer Bevacizumab und PARP(„poly ADP ribose polymerase“)-Inhibitoren als Erhaltungstherapie die Therapiesäulen. In der Palliativsituation hingegen bestimmen Aspekte der Lebensqualität den weiteren Therapieverlauf. In diesem Zusammenhang sind individuelle Therapiekonzepte, die chirurgische und systemtherapeutische Interventionen beinhalten können, notwendig.
